ICU长期机械通气患者撤机困难的因素分析

【摘要】To investigate the factors of difficulties weaning for prolonged mechanical ventilation)PMV) patients in intensive care unit(ICU), the clinical data and the weaning details of 62 PMV patients were collected and analyzed to find the factors of weaning failure. Among all patients, 32 patients weaning successfully, 10 dead, 18 weaned difficultly, among which 1 was diagnosed motor neuron disease, 2 were after brain surgery, 2 patients were after abdominal surgery, 3 were interstitial pneumonia, 3 were chronic obstructive pulmonary disease, 3 were severe pneumonia, 2 were renal insufficiency, 2 were failed to wean due to psychological stress. Case analysis displayed that the effect of respiratory and circulation factors still exist, but weren't the main reasons, neuromuscular function, psychological factors, nutrition and endocrine factors had become the main factors.%为探讨ICU长期机械通气患者撤机困难的原因,回顾性分析大连医科大学附属第一医院重症医学科的62例长期机械通气患者的临床资料,回顾撤机过程,分析其撤机成功或失败的原因.结果显示,62例长期机械通气患者,34例成功撤机;10例死亡;18例因各种原因出现撤机困难,其中运动神经元病1例,脑外术后2例,间质性肺病4例,慢性阻塞性肺病3例,普外科术后2例,重症肺炎3例,肾功能不全2例,因心理紧张焦虑
